Medical Botox for Pain
The same molecule known from aesthetics - doing serious medical work against pain
Most people know botulinum toxin (Botox) from cosmetic medicine. In pain medicine it plays a very different role: quieting overactive muscles and pain pathways that keep firing signals long after they should have stopped.
Used correctly - in the right patient, the right muscles, the right dose - botulinum toxin is an established, evidence-based tool for selected pain conditions, with an effect that lasts months per treatment.
At Team Clinic 360° in Herzliya, medical Botox for pain is administered by a specialist in anesthesiology and pain medicine, with ultrasound guidance when precision demands it.
How does botulinum toxin treat pain?
Botulinum toxin works at the junction between nerve and muscle: it temporarily blocks the release of the chemical messengers that make a muscle contract. An overactive, cramped muscle relaxes - and with it, the pain it generates.
Beyond the muscle effect, research shows the toxin also modulates the release of pain-signaling molecules from nerve endings. That is part of why it helps in chronic migraine - a mechanism that goes beyond simple muscle relaxation.
Which pain problems can it address?
- Chronic migraine - 15 or more headache days a month; treated in a structured injection protocol - see our dedicated migraine protocol page
- Persistent muscle overactivity - painful, cramped muscles in the neck, jaw and shoulder girdle that repeatedly defeat massage and stretching
- Bruxism and TMJ-related muscle pain - jaw clenching and grinding with facial pain and morning headaches
- Selected myofascial pain - stubborn trigger-point regions that respond only briefly to other treatments
- Spasticity-related pain - increased muscle tone after neurological injury, in suitable cases
Botulinum toxin is not a general painkiller, and it is not for everyone. Its power lies in careful patient selection - which is what the medical evaluation is for.
What does treatment look like?
- A medical evaluation confirming that your pain mechanism fits the treatment
- Mapping the target muscles - by examination, and with ultrasound where depth or precision requires it
- A series of small injections with a very fine needle - the session typically takes minutes
- Effect building gradually over one to two weeks
- Duration of roughly three to four months, after which treatment can be repeated
Most patients return to normal activity immediately. Side effects are usually mild and local - small bruises or brief soreness at injection points; less common effects such as temporary weakness of nearby muscles are discussed with you before treatment.
Why the injector matters more than the injection
The difference between disappointing and excellent results with botulinum toxin is rarely the medication - it is the mapping: which muscles, at which depth, at what dose.
At Team Clinic 360° the treatment is planned and performed by Dr. Lior Shem Tov, specialist in anesthesiology and pain medicine, using ultrasound guidance for deep or delicate targets, within a plan coordinated by Dr. Natalie Siegel.
When muscles will not let go - there is a medical answer
If cramped, painful muscles or chronic migraine are running your calendar, and everything tried so far brings only brief relief - it is worth finding out whether medical botulinum toxin fits your case.

Frequently asked questions
Is this the same Botox used in cosmetics?
The molecule is the same; the medicine is different. Doses, injection sites, depth and goals are entirely different - here the aim is pain control and muscle relaxation, planned by a pain physician.
How quickly does it work, and for how long?
The effect builds over one to two weeks and typically lasts three to four months. For chronic migraine, the full benefit often becomes clear over two to three treatment cycles.
Does it hurt?
The needles are very fine and each injection takes a second or two. Most patients describe the session as far easier than expected.
Is it safe?
Botulinum toxin has decades of medical use at far higher doses than aesthetics. In the hands of a specialist, at pain-medicine doses, it is considered a well-tolerated treatment; possible side effects are local and mostly temporary, and are reviewed with you beforehand.
Will my muscles become weak?
The goal is to reduce overactivity, not to disable function. Correct mapping and dosing relax the painful muscle while everyday strength is preserved.
What if it does not help me?
Then that is important diagnostic information, and we adjust the plan - dose, muscles, or a different treatment direction altogether. You will not be left with "come back in four months" as the only answer.
